Reablement Survey 2025

Overview

We would like you to help us by taking a few minutes to give us your views about the Reablement service you receive.  If you do not wish to answer the questions, this won’t affect the services you receive.

If you have any urgent concerns about your service, please contact Social Care Direct on 0131 200 2324.

If you would like, you can ask a friend or a relative to help you.  If you have a family or friend (in addition to your care worker) who is a main carer for you, there is also a section for them to fill in.

Why your views matter

The results of the survey will be used by the Health and Social Care Partnership to see how satisfied people are with their Reablement service, and to see how we can develop the service. 

Your answers will be treated as confidential and your name will not appear in any report.  However, if you raise any issues of concern, we may need to follow it up or contact you.

Respond in a different way

You can get this document on tape, in Braille, large print and various computer formats if you ask us. Please contact Interpretation and Translation Service (ITS) at its@edinburgh.gov.uk and quote reference number 25-0953. ITS can also give information on community language translations.
